Amends Iowa Code Section 812.6 to add a new category for defendants found incompetent to stand trial who pose a danger to public peace or safety and whose competency is unlikely to be restored due to cognitive or neurophysiological disability.

Requires such defendants to be committed as "safekeepers" to the Department of Corrections at the Iowa Medical and Classification Center or other designated treatment facility.

Establishes a rebuttable presumption that commitment shall continue, with annual threat assessments conducted by qualified experts and annual reports provided to the court for review.

Limits confinement to no longer than the maximum term for the criminal offense charged; when that limit is reached, the criminal complaint must be dismissed with prejudice.
