Requires Iowa health insurance policies to cover diagnosis and treatment of infertility, including up to three oocyte retrievals with unlimited embryo transfers following ASRM guidelines, effective July 1, 2026

Defines infertility as failure to conceive after 12 months of trying for women under 35, or 6 months for women 35 and older, with miscarriages not restarting the qualifying time period

Mandates coverage for standard fertility preservation services for patients undergoing medical treatments (chemotherapy, radiation, surgery) that may impair fertility

Requires fertility medication coverage to be no less favorable than coverage for other prescription medications under the same plan

Allows religious employers to request exclusion from the coverage mandate if it conflicts with their bona fide religious beliefs, but requires timely notice to employees of the exclusion
