Amends Iowa Code Section 321.210 to create a new subsection 3 specifically addressing driver's license suspension for individuals who are physically or mentally incapable of safely operating a motor vehicle.

Allows persons with physical disabilities (excluding convulsive disorders) whose licenses have been denied or suspended to obtain a special instruction permit by meeting requirements under Section 321.186 and paying required fees.

Permits holders of the special instruction permit to apply for a regular driver's license after 30 days, provided they pass all required tests including a driving test and pay applicable fees.

Waives the nonoperator's identification card issuance fee ($8) for individuals whose licenses have been suspended due to physical or mental incapacity, or who voluntarily surrendered their license in lieu of suspension.

Eliminates reinstatement fees ($20) for licenses suspended under the new physical/mental incapacity provisions while maintaining the 30-day advance notice requirement before suspensions take effect.
