Amends Iowa Code Section 598.37 to clarify name change options during divorce or annulment proceedings, allowing parties to revert to either their birth certificate name or the name they had immediately prior to the marriage.

Requires name change requests to anything other than the birth certificate name or pre-marriage name to go through the standard name change process under Chapter 674.

Mandates the clerk of court to issue a separate document evidencing the name change when a party requests one as part of a dissolution or annulment decree.

Requires the separate name change document to contain no other information relating to the divorce or annulment decree, protecting the privacy of the party requesting the name change.
