Requires physicians to perform an in-person examination of pregnant women before any abortion, including screening for signs of coercion or abuse, with referrals to appropriate health care providers as needed.

Mandates that physicians performing chemical (medication) abortions obtain signed FDA patient agreement forms and written confirmation that patients have been informed of specific risks including hemorrhage, incomplete abortion, sepsis, and sterility.

Requires patients be informed that chemical abortion effects "may be possible to reverse" and that reversal information is available on the state health department's website.

Establishes new reporting requirements for health care facilities to document complications from abortion-inducing drugs, with the data to be compiled and published by the Iowa Department of Health and Human Services.

Physicians who fail to comply with these requirements are subject to professional license discipline under the Iowa Board of Medicine.
