Requires the Iowa Department of Health and Human Services to adopt rules allowing case managers and care coordination teams to initiate transition planning for skilled nursing facility residents before discharge to the Medicaid home and community-based services elderly waiver program.

Transition planning may include needs assessments, care coordination, and service planning for residents determined capable of moving to a lower level of care and otherwise eligible for the elderly waiver program.

Expands the "targeted population" definition for case management services to include adults aged 65 or older who are nursing facility residents approved for Medicaid elderly waiver services.

Defines "assisted living service" as personal care and supportive services for elderly waiver participants in homelike, noninstitutional settings, requiring 24-hour on-site response capability for participant safety.

Establishes billing requirements specifying that one unit of assisted living service equals one day, billable only when the member was present in the facility's bed census and the provider documented at least one service encounter.
