Creates the Iowa Rural Health Transformation Fund in the state treasury under the control of the Department of Health and Human Services, separate from the general fund.

Fund will consist of federal moneys received from the Rural Health Transformation Program established under the One Big Beautiful Bill Act (Pub. L. No. 119-211).

Moneys are appropriated to the department for use as authorized by the federal Centers for Medicare and Medicaid Services, with interest and earnings credited back to the fund rather than reverting.

Department must submit quarterly reports to the General Assembly detailing all expenditures from the fund, including identifying by city where moneys were spent within the state.

Fund provisions are repealed October 1, 2032.
