Defines "cost-sharing" to include coverage limits, copayments, coinsurance, deductibles, and other out-of-pocket costs imposed on covered persons for health or medical expenses

Requires cost-sharing for supplemental and diagnostic breast examinations to be no less favorable than cost-sharing requirements for screening mammograms

Exempts high-deductible health plans (qualifying under IRS Section 223) from the cost-sharing requirements until the covered person meets their minimum deductible, except for services classified as preventive care

Applies to third-party payment provider contracts, policies, or plans delivered, issued, continued, or renewed in Iowa on or after January 1, 2026
