Creates the Iowa Rural Health Transformation Fund in the state treasury under the control of the Department of Health and Human Services, separate from the general fund.

Fund consists of moneys received from the federal Rural Health Transformation Program established under the One Big Beautiful Bill Act (Pub. L. No. 119-211).

Moneys are appropriated to the department for use as authorized by the federal Centers for Medicare and Medicaid Services, and are not subject to reversion; interest and earnings remain in the fund.

Requires quarterly reports to the General Assembly detailing all expenditures, including identifying by city where funds were spent, plus copies of all financial and performance reports submitted to federal CMS.

The fund and its provisions are repealed October 1, 2032.
