Requires the Iowa Department of Health and Human Services to adopt rules allowing case managers and care coordination teams to initiate transition planning for skilled nursing facility residents moving to the Medicaid home and community-based services elderly waiver program prior to discharge.

Expands the "targeted population" definition for case management services to include adults aged 65 or older who are nursing facility residents approved for the elderly waiver program.

Defines "assisted living service" as personal care and supportive services provided in homelike, noninstitutional settings with 24-hour on-site response capability for participant safety and security.

Establishes billing requirements for assisted living services: one unit equals one day, and billing is only permitted when the member is present in the facility's bed census and at least one documented service encounter occurs that day.

Prohibits reimbursement for assisted living services performed simultaneously with services under an approved consumer-directed attendant care agreement.
