Requires massage establishments to obtain a license from the board beginning January 1, 2027, with owners required to submit fingerprints for a national criminal history background check through the FBI.

Defines "establishment" as a location where massage therapy is practiced, excluding customer sites, self-operated massage devices, licensed health care facilities, board-approved student clinics, and sole practitioner locations.

Authorizes the board to conduct sanitary inspections prior to issuing establishment licenses and biennially thereafter, as well as upon receipt of complaints.

Establishes civil penalties up to $1,000 per offense for operating an unlicensed establishment, practicing without a license, or employing unlicensed individuals, with continued violations after board citation incurring penalties up to $10,000.

Permits the board to deny establishment license applications to massage therapists not in good standing, those with suspended licenses, or persons under criminal investigation.
