Requires all public communications about immunizations from the Iowa Department of Health, local public health departments, schools, and licensed child care centers to include information about immunization exemptions and how to obtain them.

Mandates that immunization exemption information be published on websites and included in registration documentation for elementary schools, secondary schools, and licensed child care centers.

Imposes a $1,000 civil penalty per violation on child care centers, child care facilities, child care homes, and child development homes that fail to include exemption information in immunization-related communications to parents.

Establishes a $1,000 civil penalty per violation for school districts, accredited nonpublic schools, charter schools, innovation zone schools, eligible institutions, and community colleges that fail to comply with immunization exemption notification requirements.

All civil penalty funds collected will be deposited into the state's general fund.
