Prohibits health insurance policies, contracts, and plans from imposing cost-sharing (copayments, coinsurance, deductibles, or other out-of-pocket costs) for supplemental and diagnostic breast examinations

Defines "cost-sharing" as any coverage limit, copayment, coinsurance, deductible, or other out-of-pocket cost obligation imposed by a health or medical expense plan

Creates an exception for high-deductible health plans (HDHPs) qualified under IRC Section 223, where the cost-sharing prohibition applies only after the covered person meets their minimum deductible, unless the service qualifies as preventive care

Applies to third-party payment provider contracts, policies, or plans delivered, issued, continued, or renewed in Iowa on or after January 1, 2026
