Require the Idaho State Tax Commission to add language on page two of the form below the signature lines stating "This form will not be accepted unless the information requested on line 30 is completed" or similar language.

Establish that line 30 of Form 40 relates to reporting internet purchases as part of individual income tax return filing requirements.
