Establishes an initial program fee of $25.00 and an annual program fee of $15.00 for most special license plate programs, with higher fees ($35.00 initial, $25.00 annual) for new programs effective January 1, 2013 and beyond.

Creates a new prequalification process (Section 49-402D) requiring applicants to submit a financial plan, designate a responsible individual, provide proof of nonprofit 501(c)(3) status if applicable, and demonstrate at least 250 interested purchasers.

Requires applicants to deposit estimated programming and administrative costs with the department, with a nonrefundable $1,000 administrative fee.

Mandates annual reporting by December 1 from special license plate program sponsors detailing revenues and expenditures, with suspension of plate sales on January 1 for failure to report.

Allows appeals of department denials within 20 days in writing to the department director, and requires the department to submit approved applications to legislative transportation committees annually.
