Requires physicians to perform an obstetric ultrasound before informed consent and prior to administering anesthesia or medication for any abortion procedure, with the patient having the right to view the ultrasound image and receive a physical picture if requested.

Mandates the Department of Health and Welfare publish easily comprehended, medically accurate materials describing pregnancy services, fetal development at two-week intervals from week four through week twenty-four with photographs, abortion procedures, and complications and risks to the mother.

Requires compilation of a geographically-arranged list of health care providers, facilities, and clinics offering free ultrasounds, to be included in printed materials and maintained on a stable state-sponsored internet website.

Mandates physicians or their agents sign and date a statement documenting the ultrasound test, gestational age, heart rate, and patient's acceptance or refusal of viewing the ultrasound and receiving a physical image, with the patient required to initial and sign the statement.

Establishes that knowing failure to comply with ultrasound and documentation requirements constitutes grounds for physician discipline and subjects the physician to a civil penalty of $100 per month of noncompliance, though such failure does not constitute a criminal act.
