Revises state policy to establish that individuals are responsible for their own medical care and that of their dependents, encouraging purchase of medical insurance to avoid needing assistance under the medically indigent program.

Modifies submission requirements for medical records and medical claims by hospitals and providers, requiring submission of these documents in standard format to the county clerk within ten (10) business days of notification that a patient is not eligible for Medicaid.

Establishes that findings of indigency shall start on the date necessary medical services are first provided, rather than on the date of application approval.

Changes reimbursement rate definition by extending the sunset date from July 1, 2013 to July 1, 20134 for the 95% of unadjusted Medicaid rate provision.
