Prohibits any person from knowingly facilitating use of a tanning device on minors under 14 years of age, with no exceptions.

Requires prior written informed consent from a minor's parent or legal guardian before facilitating tanning device use on minors aged 14-18, obtained in the presence of the person performing the procedure or their agent.

Defines "tanning device" as equipment emitting electromagnetic radiation between 200-400 nanometers for skin tanning, excluding personal residential devices, medically supervised phototherapy, and spray tans.

Permits physicians to authorize a minor's use of a tanning device by prescription and to use FDA-approved radiation devices for treating a minor's medical condition.

Establishes penalties of up to $500 fine for first violations and $500-$1,000 fine for subsequent violations within one year.
