Adds the Idaho Health Data Exchange to the list of entities that may receive immunization registry information upon specific request from the Department of Health and Welfare.

Revises language in subsection (4)(a) to clarify that parents or guardians can request removal of their child's information from the registry and any databases or files where it has been disclosed.

Requires that any person or entity receiving registry information may only disclose it to others in accordance with state and federal laws applicable to protected health information.

Maintains penalties of misdemeanor charges and civil damages of $100 per violation for unauthorized disclosure of registry information.
