Creates a Community Primary Care Pilot Program within the catastrophic health care cost program to provide enhanced primary care to individuals at risk of hospitalization, serving up to 1,100 eligible individuals based on available funding.

Eligible individuals must have annual income below 100% of the federal poverty level, reside in a participating county, have chronic physical or mental health conditions requiring ongoing care, and be sponsored by a nonprofit organization.

State matching funds are capped at $2.00 per dollar contributed by nonprofit organizations and individuals, not to exceed $55.00 per month per participant.

Medical services are contracted for 12-month periods and may be renewed for up to 2 additional years; individuals may remain in the program even if they exceed the poverty level threshold until their current 12-month period ends.

Program operates in five counties selected by the board (only one with population greater than 50,000) for three years, with annual reports required to legislative health and welfare committees; the program sunsets on July 1, 2019.
