Establishes the Primary Care Access Program to provide primary care management and care coordination services to uninsured adults aged 19-64 with income below 100% of the federal poverty level who are ineligible for Medicaid or advanced premium tax credits.

Creates a dedicated Primary Care Access Program Fund administered by the State Department of Health and Welfare to pay eligible providers (Idaho-licensed physicians, physician assistants, nurse practitioners, or clinical nurse specialists) for services to enrolled participants.

Requires eligible participants to complete an application, verify income below 100% of federal poverty level, confirm U.S. citizenship and Idaho residency, and undergo annual reassessment to continue enrollment based on program availability.

Providers receive monthly fees for each participant and charge participants sliding fees based on income; providers must submit utilization and clinical data to the department on a periodic basis.

Act becomes effective July 1, 2016, with participant enrollment beginning January 1, 2017, and contains a sunset provision voiding the program on June 30, 2021.
