Creates a Health Grant Program for the Medically Underserved in the state department to award grants to collect data and provide outreach and health care services.

Defines "medically underserved" as low-income persons (below 100% federal poverty level) without health insurance and not enrolled in or eligible for Medicaid or Medicare.

Requires the Community Health Center Network of Idaho to submit a report by March 15, 2018 detailing the number of medically underserved patients treated, demographic information, clinical diagnoses, and specific quality of care indicators including cancer screenings, diabetes management, and mental health services.

Authorizes the director to award one grant for fiscal year 2017 and one grant for fiscal year 2018 to the network and eligible rural health clinics for outreach and health care services, with required reporting by August 15 following each fiscal year.

Sunsets all provisions of the act on July 1, 2018, making this a temporary program subject to legislative appropriation.
