Modifies the board of trustees authority by removing language that gave them sole discretion to allocate legislative appropriations to health districts and instead establishes specific statutory formulas for distribution of state appropriations.

Establishes a three-year transition formula for state appropriations to health districts: fiscal year 2018 districts receive two-thirds (2/3) of fiscal year 2017 amounts plus proportional and poverty-based allocations; fiscal year 2019 districts receive one-third (1/3) of fiscal year 2018 amounts plus similar allocations; fiscal year 2020 and beyond districts receive 67% of pledged county contributions plus remaining balance allocated 75% by population and 25% by poverty.

Requires state appropriations to be remitted to the public health trust fund in two equal installments: one-half on or before July 15 and one-half on or before January 15.

Defines population and poverty metrics using United States Census Bureau data as of April 1 in the calendar year the appropriation is made, with successor publications determined by the board of trustees.
