Establishes the Health Care Assistance Program under the State Department of Health and Welfare to provide primary care, preventative care, and chronic disease management for uninsured individuals with income below 100% of federal poverty level who are ineligible for Medicaid or health insurance exchange tax credits.

Requires participants to meet eligibility criteria including income verification below 100% federal poverty level, U.S. citizenship, Idaho residency, health assessment demonstrating program eligibility, and verification of household composition and employment.

Requires eligible providers (Idaho-licensed physicians, physician assistants, nurse practitioners, or clinical nurse specialists) to operate under a coordinated care business model and submit utilization and clinical data to the department on a periodic basis.

Authorizes the director to establish eligibility criteria, provider reporting requirements, chronic condition limits for program sustainability, and promulgate necessary rules to implement the program.

Directs annual enrollment of eligible participants based on fund availability with a waiting list for those who cannot be enrolled immediately; requires annual reassessment for continued participation and allows providers to charge participants up to $20 per visit with providers receiving monthly fees per participant.

Program effective July 1, 2017, with participant enrollment beginning January 1, 2018, and sunset date of June 30, 2022.
