Amends Section 39-1118 (daycare immunization requirements) to clarify that religious or other grounds exemptions may be submitted as a letter from the parent or guardian rather than requiring a state-furnished form.

Amends Section 39-4802 (school immunization exemptions) to clarify that religious or other grounds exemptions may be submitted as a letter from the parent or guardian rather than requiring a state-furnished form.

Maintains medical exemptions for both daycare and school settings when a physician certifies that immunizations would endanger a child's life or health.

Specifies that exemption statements are not required to be on forms furnished by the state board of health and welfare, daycare facilities, schools, or other entities.
