Creates the Patient Caregiver Support Act establishing a framework for patients to designate unpaid caregivers to provide aftercare assistance following hospital discharge to their residence.

Requires hospitals to offer patients or their representatives the opportunity to designate one caregiver prior to discharge, and to document the caregiver's name, relationship, contact information, and any declination in the patient's medical record.

Obligates hospitals to notify designated caregivers of patient discharge and to consult with caregivers to prepare an aftercare plan including individualized explanations and in-person or video instruction on tasks needed at home.

Clarifies that caregiver designation does not obligate individuals to perform aftercare tasks, does not interfere with existing health care directives, and does not create a private right of action against hospitals or their employees.

Prohibits use of state or federal funds for caregiver payment and specifies that the chapter shall not impact state or federal program funding.
