Expands Idaho Code Section 18-1506B to criminalize practices intended to change or affirm a child's perception of their sex if that perception is inconsistent with the child's biological sex as a felony offense.

Prohibits specific medical procedures on children including surgeries (castration, hysterectomy, mastectomy, vaginoplasty, etc.), puberty-blocking medications, and supraphysiological doses of testosterone or estrogen when performed for the purpose of changing or affirming sex perception.

Exempts medical interventions for children born with medically verifiable genetic disorders of sex development, including those with ambiguous external sex characteristics or abnormal sex chromosome structures, when performed according to a parent or guardian's good faith medical decision.

Exempts surgical operations necessary to a person's health when performed by licensed medical practitioners, except when such operations impair reproductive organs solely for the purpose of changing or affirming sex perception.

Establishes that violations are felonies punishable by imprisonment in state prison for a term of not more than life, and defines "child" as any person under eighteen years of age.
