Establishes "Simon's Law" requiring physicians to notify at least one parent or legal guardian before instituting do-not-resuscitate orders, withholding artificial life-sustaining procedures, or withholding artificial nutrition and hydration for unemancipated minors.

Requires notification be provided both orally and in writing to at least one parent or legal guardian, with reasonable attempts made to notify any other parent or guardian with custodial or visitation rights.

Prohibits implementation of such orders for a minimum of 48 hours after oral and written notice are provided to at least one parent or legal guardian.

Grants parents or guardians the right to request transfer of the minor patient to another facility within 48 hours of notification, requiring the original facility to continue life-sustaining procedures for a minimum of 15 days after transfer request.

Allows such orders to be instituted if transfer cannot be arranged within 15 days, or if 72 hours of diligent efforts to contact at least one known parent or legal guardian have been unsuccessful.
