Establishes an Extended Employment Services (EES) program in the State Department of Health and Welfare to increase employment opportunities for individuals with disabilities.

Defines four categories of disability eligible for services: developmental disability, learning disability, mental illness, and traumatic brain injury, with specific diagnostic and functional criteria for each.

Requires participants to be at least 16 years old, be Idaho residents, and have a disability that creates a barrier to maintaining paid employment without long-term vocational support.

Authorizes the department to provide three types of services: individual community supported employment, group community-based supported employment (limited to 8 participants), and work services conducted on provider premises.

Requires the department to approve service providers through written agreements specifying requirements, services offered, scope of work, service fees, and other terms; allows the department to terminate approval for violations of agreements or departmental rules.
