Adds a new specialty license plate option effective January 1, 2021, allowing vehicle owners to display "Choose Life" plates in lieu of regular license plates.

Charges an initial fee of $35.00 and annual renewal fee of $25.00 for Choose Life plates, with $13.00 of each fee going to the state highway account for program administration.

Directs $22.00 of each initial fee and $12.00 of each renewal fee to Choose Life Idaho, Inc., to fund grants supporting life-affirming pregnancy resource centers and adoption services.

Restricts Choose Life plates to vehicles with a registered maximum gross weight of 26,000 pounds or less, subject to department rules and procedures.

Allows vehicle owners to transfer Choose Life plates to another vehicle upon payment of required transfer fees and receipt of new registration from the department.
