Revises Section 39-4801 to update terminology regarding immunization records and replace archaic pronouns with modern language ("his" changed to "the physician's")

Maintains existing requirement that children in Idaho schools must provide immunization records signed by a physician or licensed health care professional upon admission to preschool through grade twelve

Adds new requirement in Section 39-4802(3) that school officials must describe available immunization exemptions and provide a citation to Section 39-4802 in all communications to parents and guardians regarding immunization

Preserves two existing exemptions: medical exemption for children whose physical condition would be endangered by immunizations, and religious or other grounds exemption
