Changes immunization registry inclusion from automatic to opt-in, requiring parents or guardians to submit a specified written statement to exclude their child from the registry.

Clarifies that opt-out statements cannot be included as part of general authorizations or releases and must be standalone documents.

Maintains registry data collection including child's name, address, birth date, parent information, immunization details, provider information, and other parent-authorized information.

Preserves confidentiality protections limiting disclosure to authorized entities (health districts, schools, daycares, healthcare providers, hospitals, and the Idaho health data exchange) and establishes civil liability of $100 per violation for unauthorized disclosure.

Declares an emergency and makes the act effective July 1, 2024.
