Prohibits Idaho from implementing Medicaid expansion for adults under 65 with income at or below 133% of federal poverty level after July 1, 2026, unless 11 specific conditions are met

Requires work requirements of 20 hours per week (working, volunteering, or work program participation) for able-bodied adults, with exemptions for those under 19, over 64, pregnant, medically unfit, or caring for young children

Caps enrollment at 50,000 persons or the number of disabled/elderly Medicaid enrollees, whichever is lower, and imposes a 36-month lifetime benefit limit

Mandates federal financial participation remain at 90%, requires biannual eligibility redetermination, and prohibits funds for gender reassignment procedures or services under section 18-1506C

Department must submit waiver requests to CMS by October 1, 2025, and terminate expansion eligibility by October 1, 2026 if conditions are not met; effective date July 1, 2025
