Creates the Idaho Rural Health Transformation Fund in the state treasury to receive and distribute federal rural health transformation program grant moneys, subject to annual public reporting of all receipts and expenditures.

Establishes a nine-member Rural Health Transformation Committee with four senators appointed by the president pro tempore, four representatives appointed by the speaker, and one nonvoting member selected by the governor.

Requires the committee to establish subaward funding requirements including application scoring rubrics by March 15, 2026 for the first award year and by September 30 for the four remaining award years.

Mandates quarterly progress updates from the Department of Health and Welfare on funded projects, expenditures, and outcomes, and requires sustainability plans for all funded initiatives.

Declared an emergency measure taking effect immediately upon passage and approval; committee remains active until all fund moneys are appropriated and expended.
