Amends the Nursing Home Care Act to add definitions of "affected party" and "controlling entity" and expands the definition of "affiliate" to include entities with 5% or more ownership interests and service providers with common ownership or control.

Requires nursing home license applications to include extensive documentation: 3 years of financial data and projections, staffing information, inspection reports, affiliate and controlling entity disclosures, service agreements, and in transfer of ownership cases, detailed transaction information and regulatory history.

Establishes public notice and hearing procedures allowing residents, families, workers, unions, and consumer organizations to request hearings on license applications within 20 days, with hearings to conclude within 50 days.

Mandates that license applicants notify the Department at least 120 days (increased from 30 days) before facility ownership transfers and obtain a new probationary license before transfer completion, with violations subject to minimum penalties of $25,000 per facility per day.

Authorizes the Department to impose conditions on licenses regarding financial reporting, quality of care, staffing requirements, and additional inspections to protect resident health and safety; allows management agreement approval as a licensing condition.
