Expands the nursing home prescreening program to apply to "any persons" in need of long-term care, removing previous restrictions that limited it only to Alzheimer's Disease victims and persons deemed blind or disabled under Social Security Act.

Vests prescreening responsibility with case coordination units or any agencies designated by the Department of Human Services, broadening the entities responsible for prescreening.

Requires all persons admitted to nursing facilities who remain for 90 or more days to be re-screened at the end of the 90-day period to assess continuing need for nursing facility care and be informed of alternative care options.

Requires prescreened individuals to be informed of all care options including nursing home placement, in-home and community-based services, and their right to refuse any services.

Takes effect immediately upon becoming law.
