Amends the Juvenile Court Act and Unified Code of Corrections to impose mandatory post-incarceration requirements on gang members under 21 convicted of gang-related offenses on a first offense.

Required conditions include: community service (hours determined by court), GED completion if lacking a high school diploma, curfew compliance (11 p.m.–6 a.m. Friday-Saturday, 11 p.m.–6 a.m. Saturday-Sunday, 10 p.m.–6 a.m. Sunday-Thursday), and attendance at Life Skills classes through community colleges or non-profit re-entry programs.

Court may waive requirements only upon finding of good cause; court must inform local law enforcement of curfew requirements and order random monitoring to ensure compliance.

Defines "gang member" and "gang-related" by reference to Section 10 of the Illinois Streetgang Terrorism Omnibus Prevention Act.

Applies to both juvenile and adult offenders under 21 years of age upon completion of any incarceration term.
