Creates the Rapid Reintegration Pilot Program jointly operated by the Department of Human Services and Department on Aging to help individuals with physical disabilities, mental illness, or age 65+ who need short-term nursing facility stays (6 months or less) return to community living.

Requires hospitals in pilot program geographic areas to coordinate with Case Coordination Units, Service Coordination Agencies, and Home Services Program to identify individuals expected to have short-term nursing facility stays.

Mandates Case Coordination Units and Service Coordination Agencies to assess whether individuals need nursing facility care and whether stays will be 6 months or less, then make efforts to reintegrate them into the community.

Allows eligible individuals identified through the pilot program to receive an enhanced Community Home Maintenance Allowance for up to 6 months to help retain their homes during nursing facility stays.

Pilot program must operate for at least 3 years, with the Departments assessing effectiveness in preventing unnecessary institutionalization and supporting successful return to pre-admission living arrangements; takes effect immediately.
