Requires disabled Chicago firefighters receiving duty, occupational disease, or ordinary disability benefits to undergo medical examinations on a periodic basis as determined by the Board, but at least once annually, with exceptions for those certified as permanently disabled.

Permits the Fund to deny benefits to firefighters who refuse reasonable medical, surgical, or hospital treatment necessary to promote recovery from disability.

Establishes a repayment window for firefighters who re-entered service but failed to repay their refunds within the required 2-year period, extending the deadline to January 1, 2011; firefighters who fail to repay become new employees with service credit only from their re-entry date.

Authorizes legal action against third parties liable for injuries or deaths causing disability benefits, allowing the Fund to recover damages and maintain a lien on settlements; permits the Fund to initiate proceedings if the firefighter fails to do so within 3 months of the statute of limitations.

Exempts the mandate from requiring state reimbursement under the State Mandates Act.
