Amends the Residential Real Property Disclosure Act to add a new disclosure requirement to the residential real property disclosure report form.

Requires sellers to disclose whether they are aware of a petition, agreement, or similar document relating to the annexation of the property.

Adds this annexation disclosure as item 24 to the existing disclosure form, with yes/no/not applicable options.

Sellers must provide an explanation if they indicate "yes" or "not applicable" to the annexation disclosure question in the additional information section of the form.
