Requires group accident and health insurance policies amended, delivered, issued, or renewed after January 1, 1999 to cover diabetes self-management training and education, equipment, and supplies for type 1, type 2, and gestational diabetes mellitus.

Covers up to 3 medically necessary visits to a qualified provider upon initial diabetes diagnosis, and up to 2 additional visits if a physician determines a significant change in the patient's symptoms or medical condition has occurred.

Mandates coverage for diabetes equipment including blood glucose monitors, monitors for the legally blind, lancets, and lancing devices when medically necessary and prescribed by a physician.

Requires coverage for insulin, syringes, needles, glucose test strips, FDA-approved oral blood sugar control agents, glucagon emergency kits, and treatment for pain associated with diabetes complications.

Provides coverage for regular foot care exams by a physician or referred provider, and allows diabetes self-management training to be provided during office visits, group settings, or home visits when authorized by a physician.
