Amends the Uniform Prescription Drug Information Card Act to require health benefit plans issuing prescription drug cards to display six mandatory data elements on the front (BIN number, processor control number, group number, card issuer identifier, cardholder ID number, and cardholder name) and two on the back (claims submission information and help desk telephone numbers).

Requires new prescription drug information cards to be issued upon enrollment and reissued whenever coverage changes affect the mandatory data elements.

Establishes separate requirements for discounted health care services plan administrators, requiring their cards to include an Internet website for preferred provider lists, toll-free help desk number, provider network name or logo, group number, cardholder ID number, cardholder name (or space to write it if cardholder pays periodic charges), processor control number, and a statement that the plan is not insurance.

Applies identical uniform health care benefit information card requirements to the Uniform Health Care Service Benefits Information Card Act, covering plans providing coverage for health care services including prescription drugs or devices.

Defines "discounted health care services plan administrator" as entities other than insurers or HMOs that arrange contracts with providers offering incentives to use their services without other cost-sharing differentials, or that contract to enroll beneficiaries in independently identifiable preferred provider programs.
