Amends the Illinois Animal Welfare Act to require dog dealers and cattery operators to provide specific information about dogs and cats for sale, including age, breed, vaccination records, sterilization status, and breeder information.

Establishes new disclosure requirements for animal shelters and animal control facilities offering dogs or cats for adoption, including breed, age, medical treatment received, adoption fees, and license number, with information posted on or near animal cages.

Establishes new disclosure requirements for pet shop operators selling dogs or cats, including retail price, breed, medical treatment, breeder information and license numbers, congenital or hereditary disease information, and pedigree registry details.

Requires all disclosures to be provided in written form with signed acknowledgement forms from both the seller/facility and the buyer/adopter, with original copies maintained for 2 years.

Takes effect January 1, 2011.
