Requires individual and group health insurance policies and managed care plans to cover hearing instruments and related services when prescribed by a licensed audiologist or physician for all individuals.

Mandates coverage up to $2,500 per hearing aid per impaired ear, subject to applicable co-payments, deductibles, co-insurance, and out-of-pocket limits.

Allows hearing aid replacement once every 36 months as prescribed, or at any time if there is a significant change in hearing status (defined as 10 decibels HL change on three-frequency pure-tone average).

Requires coverage of related services including audiological exams, replacement ear molds, and hearing aid repairs at all times when prescribed by a hearing care professional.

Prohibits insurers from paying duplicate claims if an identical claim was paid by any insurer within the prior 36 months; effective immediately upon passage.
