Requires Secretary of State driver services facilities to ask applicants who will be at least 18 years old at the next election if they wish to register to vote when applying for original, duplicate, corrected, or renewal driver's licenses or identification cards (except temporary visitor's licenses).

Applicants who wish to register must complete a voter registration application that the Secretary forwards to the appropriate local election authority; those who decline must sign a declination statement.

Establishes a Secretary of State database to maintain records of all persons who complete voter registration applications at driver services facilities.

Applicants are responsible for determining their own legal eligibility to register to vote.

Requires driver services facilities to provide brochures to applicants without charge warning of financial identity theft dangers, prepared by the Department of Financial and Professional Regulation.
