Requires physicians or qualified ultrasound technicians at abortion facilities to offer women seeking abortion after 6 weeks of gestation an opportunity to receive and view an active ultrasound of the unborn child at least one hour before the abortion procedure.

Ultrasound must be performed by a qualified person with documented ultrasound training and must be of quality consistent with standard medical practice.

Abortion facilities must document each woman's response to the ultrasound offer in writing, including the date, time, and the woman's signature indicating whether she accepted or declined.

Department of Public Health must prepare annual reporting forms for abortion facilities to submit data on the number of women who chose to view ultrasounds versus those who declined, and whether they proceeded with abortion.

Medical emergency exception allows physicians to forgo ultrasound requirements based on their good faith clinical judgment that an immediate abortion is necessary to prevent death or serious bodily impairment.
