Requires the Secretary of State to issue Voter Identification Cards to registered voters who lack acceptable photo identification, valid as long as voter maintains the same name and residence address.

Defines acceptable forms of photo identification for voting purposes as Illinois Driver's License, State Identification Card, Disabled Person ID, Senior Citizen ID, FOID Card, U.S. Passport with current address, and other government-issued photo ID with name, current photograph, and current address.

Mandates voters present either a government-issued photo identification card (as defined in Section 3-6) or a Voter Identification Card to election judges for verification of identity before voting in person or by early ballot.

Establishes exemptions allowing indigent voters (income at or below 125% of federal poverty guidelines) and voters with religious objections to being photographed to cast provisional ballots and sign affidavits instead.

Requires provisional voters who registered by mail and fail to provide acceptable photo identification to have 10 days to submit the required identification card to the county clerk or board of election commissioners.
