Defines "catastrophic injury" as a grievous or serious injury or impairment that permanently prevents an injured employee from performing any gainful work.

Requires employers to pay entire health insurance premiums for law enforcement officers, correctional officers, firefighters, their spouses, and dependent children (up to age 25 if students) who suffer catastrophic injury or are killed in the line of duty.

Allows employers to require employees seeking benefits to submit to examination by up to 3 licensed physicians at the employer's expense.

Makes catastrophic injury determinations final when made by the employer's corporate authorities or designated person, subject only to judicial review under the Administrative Review Law, with the employer designated as a necessary party.

Establishes that making false or misleading statements to obtain coverage is a Class A misdemeanor, resulting in forfeiture of benefits and reimbursement of all paid benefits if convicted.
