IL HB4484

Amend House Bill 4484 to require that individuals must have previously failed to attend an appointment for which notice was provided at least 5 days before the appointment.

Add requirement that individuals must be notified of the time and place of the appointment at least 5 days before the independent medical examination.

Renumber existing provisions (2) and (3) to become provisions (3) and (4) respectively due to the addition of new requirements.
