Department must investigate and report on deaths or serious life-threatening injuries of children in its care or involved in abuse/neglect reports to the central register.

Reports must include cause of death/injury, circumstances, services provided to the child/family in preceding 5 years, Department actions, policy recommendations, criminal charges against alleged perpetrators, and copies of investigation documents and files.

Department Director must consult with State's Attorney and release reports to public upon request, with redactions allowed only for mental health information, attorney communications, reporter identity, information harmful to siblings, ongoing criminal investigations, and federally prohibited disclosures.

Department must notify legislative leaders and affected district representatives within 6 months of each death or serious injury and submit annual cumulative reports to Governor and General Assembly for public availability.

Department may request and must timely receive information from state agencies, political subdivisions, and other entities that provided services to the deceased or injured child.
